Files must be submitted in one PDF in the following order:
- Title Page: to include Name, School/Hospital affiliation, Title of Project, Mentor’s name and email, Program Director’s name and email, Department Chair’s name and email
- 4 page summary with title, background information, project description and goals, potential significance, proposed budget, estimated time commitment, acronym definition list
- Bibliography of the project
- IRB or IACUC approval, or proof of application
- Applicant’s CV
- Letter of support from Mentor
- Mentor biosketch
- Letter of nomination from residency of fellowship program director, including a commitment to allow time of complete project
- Letter of endorsement from department chair
